We, Can Medikal Hizmetler A.Ş. Özel Lifemed Tıp Merkezi, taking into consideration the safety of our valued customers and patients, we would like to inform you about the "Personal Data Protection Law in which is regulated with regard to the protection of fundamental rights and freedoms, especially the privacy of private life, and to fulfill our "Law on Protection of Personal Data" within this scope.
Our aim; to provide you with the most transparent information about the way your personal data is received, the purposes for which it is processed, the legal reasons and your rights.
By virtue of the Personal Data Protection Law No. 6698 has been enacted on the date of April the 7th 2016, our company thus in the capacity of Data Officer and Data Processor, personal information; within the framework described in the law and adhering to the conditions and limits described below; obtain, save, store, update, modify, reorganize, disclose, transfer, share, classify, anonymize within the scope of the purpose and scope described below e.g.Basic Law of Health Services No. 3359, Health Services Law No. 663, the Decree Law on the Organization and Duties of the Ministry of Health and its Affiliated Organizations, Regulation on Outpatient Diagnosis and Treatment, Law No. 6698 on Protection of Personal Data, Regulation on Personal Health Data, regulations of the Ministry of Health and other relevant legislation and other forms enumerated in the law.

1. Personal Data Processed
Your personal and private personal data, especially your health data: name, surname, TR Identity Number and/or passport number and/or temporary TR ID number, place of birth and/or marital status, gender, health insurance, profession, your insurance card number, business register and/or patient identification number, business register and/or patient identification number and any other identification data that may identify you; your address, telephone number, e-mail address and other contact data, personal contact data obtained by customer representatives and/or patient services by virtue of call center standards, and personal data obtained when you contact us by e-mail, letter and / or other means; your financial data such as billing information; your data on private health insurance for the purpose of financing and planning health care services and your data on payment institution information such as the Social Security Institution; patient medical reports, diagnostic data, biometric and genetic data, laboratory results, test results, examination data, physician analysis and comments, appointment information, prescription information including but not limited to during and / or during the execution of medical diagnosis, treatment and care services all kinds of health information and data obtained as a result, your notifications such as survey, thank-you letter, complaints letters, satisfaction results; if you use the car park, your license plate information, as required by the legislation in our medical center, the images obtained from the camera records that are constantly recorded in public areas, Özel Lifemed Tıp Merkezi; all personal data related to your health data, IP address, cookie and other personal data, resume provided in this case, and any personal data related to your service contract, which you submit and / or enter through all web sites and online services of all information and documents that serve to make the identity of our customers / patients specific or identifiable are within the scope of personal data pursuant to Article 3/d of the KVKK ( “Personal Data”)
Your personal data can be processed by Özel Lifemed Tıp Merkezi by virtue of the purposes stated in Article 2 and can be transferred to the persons , institutions and organizations stated in Article 3 within the scope of your Express consent or the reasons specified in the relevant legislation.
2. Purpose and Legal Reasons of Personal Data Processing
Your Personal Data may be processed by Private Lifemed Medical Center fort he following purposes:
- The Basic Law of Health Services No.3359, Decree Law No.663 on the Organization and Duties of the Ministry of Health and Affiliated Organizations, Regulation on Outpatient Diagnosis and Treatment Law No.6698 on Protection of Personal Data, Regulation on the Processing and Protection of Personal Health Data in other legislation and to fulfill our legal obligations and the requirements of the services you request.
- Identification and verification in order to prevent the capture of your Personal Data / Personal Data by others.
- Public health protection, preventive medicine, medical diagnosis, treatment and care services, health services and financing, planning and management.
- Planning and managing Daily operations and internal operations of our medical center.
- Supply of medicines and/or medical equipment and/or devices specific to you.
- If you make an appointment, be able to inform you about the appointment, provide information and/or make an appointment reminder.
- Performing risk management and quality improvement activities.
- Fulfillment of legal and regulatory requirements.
- Sharing and responding with the Ministry of Health and other public institutions and organization by the virtue of provisions of the legislation.
- Inquire of your entitlement with the institutions / organizations contracted with our medical center or provide financial agreement with these institutions regarding the health services provided to you.
- Sharing and authenticating the information requested by the contracted institutions / organizations, especially private insurance companies, in the financing of health services.
- Your payments can be realized, e-fatura, e-arşiv faturalama and if necessary, return and change transactions can be made, invoice in return fort he services we offer,
- Taking all necessary technical and administrative measures within the scope of data security of the systems and applications of our medical center.
- Analyze your use of health services and store your health data and respond to your questions or complaints about our services in order to improve the health services we offer you.
- Sharing information to you through your preferred communication channel within the scope of treatment and service, determination of transaction information.
- Provision of necessary information in line with the demands and inspections of regulatory and supervisory authorities and official authorities.
- Maintaining information about your health data that should be kept by the virtue of the relevant legislation.
- Compliance with internal policies and principles.
- Measuring patient satisfaction and increasing patient satisfaction after receiving health services,
- Transfer and follow up of your complaints / requests to our company – if you have given permission to communicate, we will be able to provide you with special promotion and information activities, to make marketing and campaign activities, to send bulletins / brochures, to analyze data-research services, planning and management of health services and financing, increasing patient satisfaction, researching and related reasons, to contact you for information purposes, including, but not limited to medical diagnosis, treatment, and care.
- Backing up, archiving, storing and storing the transaction records made, keeping information, reporting, informing obligations complied with the provisions of the relevant legislation and performing our legal obligations.
Your personal data obtained and processed by virtue of the relevant legislation can be transferred to the physical archives and/or information systems of Özel Lifemed Sağlık Merkezi and kept in both digital and physical environment within the period specified in Article 4.
3. Personal Data Transfer
Ensuring that all necessary technical and administrative measures are taken to ensure the appropriate level of security by virtue of KVKK and the relevant health legislation, and in accordance with the purposes of Chapter 2; Basic Law of Health Services No.3359, Decree Law No. 663 on the Organization and Duties of Ministry of Health and Affiliated Organizations, Regulation on Outpatient Diagnosis and Treatment Law No.6698 on Protection of Personal Data, Regulation on Personal Health Data and other relevant legislation / or organizations; depending on the payment method you prefer, the relevant Banks, online paymen systems, the insurance / foundation / fund, if you have received services within the scope of the insurance company, foundation, fund and intermediary institutions, to resolve complaints and provide you with a better service to the infrastructure provider, e faturalama / e-arşiv faturalama to the relevant institutions / organizations and infrastructure providers, back up / storage / archiving / storage of the transaction records for the server hosting, archiving, storage and software services providers; Our Company’s employees, legal, financial, and tax advisors, auditors, and the persons, institutions and organizations allowed by the provisions of the legislation shall be transferred to the Company fort he purpose of eliminating your complaints or a problem that arises or performing our legal obligations. When transferring your personal data to third parties referred to in this article, a transfer shall be made only to the extent necessary, and within the scope of its relevance.
4. Method of Collecting and Storing Personal Data

Our company shall ensure that the personal data provided by the company comply with the relevant periods in the event that there is a prescribed period under the provisions of the legislation to which it applies; if such a period is not foreseen, they shall only be able to keep it for as long as necessary for the purpose for which they were committed, Your Personal Data (KVKK) Article 7 / Item 1 shall be deleted, destroyed or anonymized when the purpose for processing according to Article 7 / Item 1 has been removed and/or the statute of limitations required by us to process your data has expired.
5. Rights and Exceptions to the Personal Data Holder
By applying to our Company in writing or by other methods determined by the Board; a) find out whether your personal data has been processed, b)request information about your personal data if it has been processed, c) find out whether your personal data has been used for its intended purpose, e) Requesting the deletion or destruction of your personal data by virtue of provisions of Article 7 of the Law, g) to object to a conclusion against you that your personal data is analyzed exclusively by automated systems; and i) to claim damages if you suffer damage due to unlawful processing of your personal data.

It is possible to exercise your rights as of 07.10.2016, the effective date of the regulation. You can send it to the address via registered mail, notary public or by hand delivery method with a petition / application letter. In addition, if the Board determines other methods for the transmission of the request, you will be able to submit your request by such methods. Our company shall finalize your requests in the application as soon as possible according to the sort of the request and within thirty days at the last.
However, if the transaction requires additional cost, the fee in the tariff determined by the Board may be charged. Our company has the right to accept the request or to refuse by explaining the reason and to inform the person concerned in writing or electronically. If the request in application is accepted, our company shall fulfill your request. If there is a fee in case the application is due to the mistake of our company, the fee shall be returned to the person concerned. In case of rejection of the application, inadequate response or failure to respond to the application in due time; the right of the person to complain to the Board within thirty days from the date of learning of our company’s reply and in sixty days from the date of application. By virtue of Article 14 / 2 of the KVKK, the company cannot apply to the Board of complaints unless the application path is exhausted. Persons whose rights are violated, shall be entitled to compensation under the general provisions.
Notwithstanding, pursuant to Article 28 / 2 of the KVKK (Law on Protection of Personal Data); except the right to claim damages;
a) Personal data processing is necessary fort he prevention of crime on for criminal investigation.
b) Processing of personal data publicized by the person concerned.
c) Personal data processing is required for the conduct of supervisory or regulatory duties and for disciplinary investigation or prosecution by authorized and authorized public institutions and organization and professional organizations in the nature of public institutions based on the authority granted by law.
d) We also state that personal data processing shall not apply where it is necessary to protect the economic and financial interests of the State in relation to budget, tax and financial matters.
6. Where Personal Data / Personal Data can be processed without explicit consent in accordance with KVKK
Pursuant to Article 5 of the KVKK, your Personal Data may be processed without your Express consent in the following cases:
- Where expressly provided for by law.
- If you are unable to disclose your consent as data owner due to the actual impossibility or where your consent is not legally valid, your personal data must be processed to protect yourself or someone else’s life or body integrity.
- The processing of your personal data of the parties to the contract, provided that it is directly related to the establishment or bankruptcy of a contract.
- It is compulsory to fulfill a legal obligation.
- Your personal data is publicized by you.
- Data processing is mandatory for the establishment, use or protection of a right.
- Providing data for the legitimate interests of Can Medikal Hizmetler A.Ş. is compulsory, provided that it does not harm your fundamental rights and freedoms.
- In addition, your personal health data may be processed by persons under the obligation of secrecy or authorized institutions for the purpose of protection of public health, conducting preventive medicine, medical diagnosis, treatment and care services, planning and management of health services and financing without seeking the express consent of the person concerned.
In addition, there is no need to obtain explicit consent of the disclosure, use and transfers to be made to the persons / institutions who may request information due to legal obligations and / or to fulfill the legal obligations arising from the legislation that our company is subject to.
